Critical Thinking: a Lost Art

Some things to unpack here.
Manufacturers of silicone denture soft liners like to promote that their liners are capable of lasting 4 or more years before replacement over plasticized resin soft liners which harden and peel about 12-24 months.
On the surface that seems like a great thing except I think that there are some real drawbacks to promoting this benefit.
Because of the longevity property, patients are not motivated for recall, especially if the promise of longer liner life is made. By and large these patients are typically poor “recall observant” candidates to begin with.
As this study indicates denture teeth wear faster than the silicone liners erode. If the ridge continues to remodel naturally and/or is exacerbated by medications and metabolic conditions and the fit and function evaluations(recalls) are lengthened between visits, it only makes sense that people have a better chance of developing parafunctional conditions which will promote faster tooth wear and further parafunctional compromises.
In my mind it would be better to have a plasticized resin soft liner that hardens and peels in 12-24 months which has a better chance of prompting the patient to observe recall and the clinician a better chance of heading off potential further parafunctional problems which will destroy bone, fit, and function.
A Quintessence book I can suggest, which has aided me over the years to make sound recommendations in my capacity as a dental technician, product developer, manufacturer consultant and educator.
