The start of a case always involves the evaluation and communication of available space to support my suggestions and the rationale behind them. Photos, articulations, dialogue, and use of measurement tools all come into play.
Client and patient wanted a more fixed prosthetic, but patient couldn’t afford implant borne retained attachments presently, but dentist client wanted to preserve bone for the eventuality of implant placement. Easy task once I can make a sound suggestion for attachment modality.
My suggestion after discerning vertical and radicular space was the old time but still viable interradicular ZAAG attachment. Rationale being that because this attachment lows the fulcrum to a sub-gingival location it would be much kinder to the natural abutments into which they would be placed. This will hopefully buy enough time over the long run to allow for bone preservation and time for the patient to recoup finances and realize the benefit of a retained denture over a non-fixed version.
